Registration Requirements
The following is needed to be able to register your child:
- 3 types of Proof of Address (Child must live in Bonsall district)
- Original Birth Certificate (Must have both parents names / NO COPIES)
- Immunization Records (*Please see below)

3 DPT & 1 dose on or after 4th birthday 4 DPT
2 Polio & 1 dose on or after 4th birthday 3 Polio
1 MMR 2 MMR
1 HIB 1 HIB
3 HEP B 3 HEP B
1 VAR & 1 dose on or after 1st birthday 1 VAR & 1 dose on or after 1st birthday
Prenar (PCV) 1 dose on or after 1st birthday
Influenza (6 mos. - 59 mos. old) - 1 dose of influenza between Sept 1 - Dec 31
